Registration Fees and Deadlines

Confirmation

All participants will receive letter of Confirmation via e-mail or fax within 3 working days. The registration process will accelerate by enclosing a copy of a bank transfer or a credit card authorization form to your application form.

In case of difficulties during the registration process please contact the Symposium secretariat at tsmch@guarant.cz.

Tax document will be issued on demand after the conference, further details will be given at the registration desk.

Early registration fee will only be accepted if the Registration Form and payment are received by the Congress secretariat by 31st July 2010 at the latest.

Standard registration fee will only be accepted if the Registration Form and payment are received by the Congress secretariat by 31st August 2010 at the latest.

Late registration fee will be applied after 1st September 2010 and on site.

Registration fee includes:

• Admission to Scientific Session
• Admission to the Poster Area
• Admission to the Commercial Exhibition
• Congress materiále (badge, programme, book of abstracts)
• Refreshments during the symposium (coffee breaks)
• Social event

Registration fee does NOT include:

• Participation on pre/post-congress course

Pre/post-congress course fee includes:

• entrace to a full day course
• possibility to participate on practical surgery

Cancellations and Refunds

The Symposium Secretariat must be notified in writing about the cancellation of registration by fax +420 284 001 448 or by e-mail tsmch@guarant.cz. Telephone amendments will not be accepted. The appropriate refunds will be made after the Symposium. The following cancellation conditions apply:

Cancellations received till 31st July 2010 – 50% refund
Cancellations received after 1st August 2010 - no refund

Terms of Payment

Registration fees must be paid in EUR by:

Credit Cards

Only Mastercard/Eurocard, VISA, Diners Club and American Express credit cards are accepted.

According to the Czech law, credit cards will be charged in local currency – Czech crowns (CZK) by using the exchange rate of the Czech National Bank on the date of payment. The approximate exchange rate is 1 EUR = 26,00 CZK.

Bank Transfer

Please transfer the payment in EUR after receiving a Letter of Confirmation which will be sent to you as a confirmation of your registration to:

Please note that all bank charges must be fully covered by the participant.

A copy of the bank transfer must be sent together with the Registration Form.

Name change

If you should be prevented from attending, you will be given the opportunity to send a colleague instead of you. Symposium secretariat must be notified in writing about any name change. Handling fee of 30 EUR will be charged for each name change.
